The answers to all the below questions including all information and particulars given herein are accurate, true and complete and are given to the best of my/our knowledge and belief.
I/We have not withheld any material information and accept that this application and declaration shall from the basis of the contract between the Company and me/us.
I/We hereby acknowledge that failure to supply true and accurate answers to this application or inform the Company of all material information about my/our application may render the Company unable to accept or process this application or the insurance policy void.
The insurance coverage applied for shall only take effect when this application has been accepted by and the first premium has been paid to the Company.
The Insured Person must legally reside in Hong Kong.
The monthly income of the Insured Person must be within HKD4,000.
The age limit of the Insured Person is 18 – 65.
Exclusion: non-domestic duties, vehicle driver, gardener and post-natal care helper.
The employees' compensation insurance cover employers' liabilities under the Employees' Compensation Ordinance and the common law. Employers should decide on the type of employees' compensation insurance policy for their MaMaria Helpers according to the length of employment period. Employers have the sole discretion to choose any insurer regardless of the kind of insurance policy to be taken out.
The Employees' Compensation Ordinance applies to all employees who are employed under contracts of service or apprenticeship, which includes MaMaria Part-time Helpers irrespective of the mode of employment. According to Section 40 of the Employees' Compensation Ordinance, no employer shall employ any employee in any employment unless there is in force a policy of insurance to cover their liabilities both under the Ordinance and at common law for injuries at work in respect of all their employees, irrespective of the length of employment contract or working hours, full-time or part-time, permanent job or temporary employment. Should there be any enquiries on legal liability, employers should seek professional legal advice.
